A Prescription Format
Prescription is the basic formula on which the preparation is made. it is a written order by a physician to a pharmacist, requesting him to make a certain pharmaceutical preparation in a certain form and in a certain quantity to be given over to a patient.
It is the duty of a pharmacist to prepare medicines using his scientific knowledge and experience and hand over to the patient elegant preparations with clear instructions as to how medicines should be taken.
Since a prescription is a written order a pharmacist should not alter a prescription without the authority of the prescriber. whatever alterations, he must authorise in writing with his initials against all such alterations.
The Following four parts could be identified in all prescriptions and for purposes of dispensing all these four parts are essential.
01. Rx Recipe, or the Superscription
Here the Rx denotes Recipe meaning 'take thou' which may be interpreted as Take the medicine. This is the first part of the prescription. it is a symbol signifying Dispensing and is used traditionally all over the word, except in France, where they use p meaning Pranez or Take.02. Inscription : It's the body of the prescription.
It contains the various ingredients that go to make the preparation, with the quantities per unit or dose of the preparation.
03. Subscription
Here the instructions by the physician to the pharmacist are given. The instructions are to mix and make a certain preparation in a certain form and in a certain quantity to be given to a patient.
e.g. mft mist mittemice fiat mistura mitte 120 mL
mix and make a mixture and send 120 mL
The middle term indicates the requisite preparation.04. The signature or the Label : S/- (Symbol).
Here are the instructions by the physician to the patient, how to take or use the medicine.
e.g. S/30 m. t.d.s., a.c.Label : Take two table spoons full three times a day before meals.
